Jack has never died and stayed that way, except maybe once. While most of us (including me) believe that Jack=Face of Boe, R. T. Davies and S. Moffat have refused to explicitly state that they are the same. I suspect this is to help us feel what the Doctor must be feeling, that is certainty in the face of ambiguity. Joy.
